This Initial Physical Therapy Evaluation will be to perform a comprehensive movement and strength assessment, discuss what brought you into physical therapy, your goals and formulate your plan of care moving forward.
Follow up Cash Pay Option -$160/ 30 minute follow up $80
The follow up appointment will occur after the initial evaluation at a frequency determined by you and Dr. Logan. This follow up will be used to monitor your progress and perform any necessary changed to your plan of care.
