What do I need to bring to my first appointment?

• A valid Photo ID

• Insurance Card

• MD Referral (if provided)

• Current list of medications and any relevant past surgical history

What do I need to wear?

• Comfortable loose fitting clothes that will allow for easy movement and access for the physical therapist to treat the affected area.
